Reason for ban: Its from the thread with the words "READ FIRST" next to it. After that, edit your first post and remove the "Not unbanned" because it makes them think you weren't unbanned for a reason and sometimes they just skip over the unban request. Playing with the prefixes might prevent you from being unbanned because Staff might not check your post because it already looks like somebody has handled it.
