wait a sec the cop threatened to give you the improper lane change ticket? That is messed up and I would have asked for her supervisor.

 

And yeah never fill anything out especially if it is from someone else insurance company. Read the entire thing carefully and also find a branch for your insurance and show them the paper as well.
