Constant disrespect of me and other players, spam of chat was muted 5 times, kicked, and banned, even more abuse on mic
Then comes on teamspeak and spams pokes
Can post demo later when needed.

In game ban stays at 2, i dont see much proof that make me want to extend in game ban.
For ts3 you have to get his unique id and post ban report here http://hellsgamers.com/forum/166-ban-reports-and-requests-for-non-steam-bans/

Edit: by chat logs he deserves a ban.
