Most of these are registry files (one is zipped up) and all oyu have to do is double-click and merge the registry file (this is automated by the .reg file)

 

Second, goto Start -> Control Panel -> Default Programs -> Associate a File Type, you can manually pick them off there. I don't know why you need to use registry to do it. lol unless .cpl is broken too. If your EXE file extensions are broken, follow my first link provided. It should fix it.

 

in Safe mode, you can try running sfc /scannow in CMD.exe. This usually looks for inconsistencies in core Windows files.

Last case is always to do a reinstall but I'm surprised that System Restore didn't kick in :/

uhm I think I know what virus you have is it popping up with any scanner like a virus scanner?

 

It doesn't sound like any rogue software. I would hope by now he would of mentioned some big ol' box is in the middle of my screen that I cant remove and I dont ever remember installing. lol
